Assumption of Risk: I am fully aware that participating in exercise classes, massage therapy, and athletic recovery services involves inherent risks, including but not limited to physical injury, strains, sprains, and the rare possibility of serious injury or death. I voluntarily assume all risks associated with my participation in these activities. I confirm that I am physically and mentally capable of participating in the activities offered by the Facility.

Medical Clearance: I acknowledge that I have consulted with a medical professional and have received medical clearance to engage in physical activities and services offered by the Facility. I will promptly inform the Facility’s staff of any changes to my medical condition that may affect my ability to safely participate in the activities and services.

Release of Liability: In consideration for being allowed to participate in the activities and services provided by the Facility, I hereby release, waive, discharge, and covenant not to sue the Facility, its owners, employees, contractors, agents, and any affiliated individuals or entities (collectively referred to as “Released Parties”), from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in the activities and services, whether caused by the negligence of the Released Parties or otherwise.

Indemnification: I agree to indemnify and hold harmless the Released Parties from any liability, claims, demands, actions, or causes of action brought by third parties arising out of my participation in the activities and services provided by the Facility.

Photography and Likeness Release: I consent to the use of my photograph, image, likeness, or video for promotional, marketing, or educational purposes by the Facility without compensation or further notice.

Severability: I agree that if any provision of this Waiver is found to be unenforceable or invalid, that provision shall be severed from this Waiver, and the remainder of the Waiver shall continue to be valid and enforceable.

Governing Law: This Waiver shall be governed by and construed in accordance with the laws of the State of Virginia in the United States of America without regard to its conflict of law principles.

Cancellation and No Show Policies

For ALL SERVICES, less than 24 hours of canceling / rescheduling will result in loss of a session of any service packages, or 50% of the cost for single session purchases.

No Show Policy:

No Show will result in loss of a session of any service packages, or 100% of the cost for single session purchases.

Refund policy:

Elite Wellness does not refund any purchases. We offer funds to be used towards another service purchase(s) for yourself or another person.

This information is provided in the email conformation of any purchases and appointments. Clients are responsible to review. It is also provided as an agreement in our registration portal and the client is responsible to review. Making purchases and appointments without signature, assumes that client has agreed.
